Mass Dept of Revenue Sales Tax Resale Certificate Form ST-4

This certificate is to be used when the purchasing company intends to resell the tangible packaging, packaging supplies, or services in the regular course of business.

Mass Dept of Revenue Sales Tax Exempt Purchaser Certificate Form ST-5

This certificate is to be used by 501(c)(3) organizations that have received a Certificate of Exemption (Form ST-2) from the commissioner of Revenue certifying that it is entitled to exemption under G.L. c64H.

Mass Dept of Revenue Exempt Use Certificate Form ST-12

This certificate is to be used when the purchasing company intends to incorporate the tangible packaging, packaging supplies, or services into the manufacture of their product or services.
